Exercise-induced myocardial ischemia. Importance of coronary blood flow.

چکیده
T he onset of ischemia is thought to occur when myocardial oxygen demand exceeds supply. In an individual patient the ischemic threshold might be a reproducible measure that could be used in the short term to establish the efficacy of therapy. In clinical studies the operational definition of the onset of ischemia has been 0.1 mV of ST depression on the electrocardiogram. In earlier investigations it was assumed that coronary supply was relatively fixed in patients with chronic ischemic heart disease and that changes in the onset of ischemia during exercise were mainly determined by changes in demand. Traditionally demand has been estimated by heart rate or the rate-pressure product. Clinical investigations have shown relatively constant rate-pressure products at the time of onset of ischemic ST segment changes; these observations confirmed the theory that supply was relatively fixed and explained the efficacy of agents that lowered demand, such as /3-blockers.' However, in individual patients the test-to-test variability of rate-pressure product at the time of ischemia onset may be great.2

Persistence of regional left ventricular dysfunction after exercise-induced myocardial ischemia.
To determine whether regional myocardial dysfunction occurring after exercise-induced ischemic might be caused by continued abnormalities of myocardial blood flow in the post-exercise period, nine dogs were instrumented with ultrasonic microcrystals for determination of circumferential segment shortening, circumflex artery electromagnetic flow probes, and hydraulic coronary artery occluders. Do...
متن کاملAttenuation of exercise-induced myocardial ischemia in dogs with recruitment of coronary vasodilator reserve by nifedipine.
There is now evidence that under resting conditions coronary vasodilator reserve exists even in the presence of myocardial ischemia. Therefore, we tested the hypothesis that a vasodilator reserve may exist during exercise so that during exercise-induced ischemia a reduction in coronary constrictor tone can be produced that attenuates the decreases in regional myocardial blood flow and function ...
متن کاملMechanism of beneficial effect of beta-adrenergic blockade on exercise-induced myocardial ischemia in conscious dogs.
We examined the importance of decreased heart rate in the beneficial effect of beta-adrenergic blockade on exercise-induced regional myocardial ischemia and contractile dysfunction in conscious dogs with single vessel coronary stenosis (ameroid constrictor). Studies were performed during control treadmill exercise, which produced regional myocardial ischemia (blood flow measured with microspher...
متن کاملSerial Myocardial Imaging after a Single Dose of Thallium-201
Although thallium-201 exercise scintigraphy has been established for the detection of myocardial ischemia and viability, little is known regarding the myocardial thallium-201 kinetics during angioplasty. Herein, we report a 77-year old man with angina pectoris, in whom serial myocardial imaging after a single dose of thallium-201 was helpful in identifying not only the culprit lesion and myocar...
متن کاملMyocardial stunning in exercise-induced ischemia in dogs: lack of late preconditioning.
Late preconditioning (PC) against myocardial stunning develops after coronary artery occlusion (CAO) at rest and subsequent reperfusion. We investigated whether late PC occurs after exercise-induced ischemia (high-flow ischemia) in dogs. A circumflex coronary artery stenosis (by using occluders) was set up before the onset of treadmill exercise in nine chronically instrumented dogs to suppress ...
